| Description |
The service has Outreach Workers based in South Wye and Leominster (two in each area). Their work is multi-faceted from offering impartial, informal advice and information, to supporting young people in a non-judgmental way with relapse prevention, harm reduction and complete abstinence advice. They are qualified to offer help and assistance on matters of sexual health, housing and employment.
Offering face to face 2:1 support, workers act as 'sign posters', referring young people to other agencies.
DASH, the county treatment provider is the mother organization of the Outreach Project. |

| To make contact with difficult to reach young people and those not engaged in services, to steer them in the right direction and to try and reduce drug related harm. |

| Many multi-agency links such as CAMHS, Youth Counselling Trust, Alcohol services. If they need to refer a young person on to another agency, they will become the Key Worker and plan and coordinate a care package for that young person. |
