Method of Access: Referrals can come from any agency, parents/carers or the young person.
Description
A referral form is completed and sent to the Inclusion Support Worker, who will initiate contact with the referrer and the family to explore the nature of the problem(s).

Using the ONSET screening tool, the worker will then make an assessment of the young person’s problems/needs.

Information is collated and presented to a multi-disciplinary panel that meets on a regular basis. An action plan is then drawn up and a lead agency is identified to implement and review the plan.

Interventions that might be offered include: parenting skills sessions, mentoring for the young person and/or parents, engagement of the young person in sports/leisure activities e.g. PAYP, Positive Futures.

Review meetings are held regularly to monitor progress.

The project has also developed a range of support especially for parents:

• Systemic family therapy in Stratford & Warwick districts
• Family counselling in partnership with RELATE in Rugby, Warwick, Nuneaton & Bedworth
• Riding the Storm parenting group
• Use of parenting Wisely CD Rom
